Acceptance of these terms
These Terms and Conditions apply to your use of LeadSigma Scheduling pages and related appointment scheduling services. By using the scheduling service, submitting an appointment request, or checking a consent box, you agree to these terms and to any additional terms or notices provided by the healthcare practice you are contacting.
Scheduling service
LeadSigma Scheduling helps healthcare practices receive appointment requests, intake details, and scheduling preferences. Submitting a request does not guarantee that an appointment is confirmed unless the page or practice confirms it. The practice may contact you to verify information, reschedule, cancel, or request additional details.
No medical advice or emergency use
Scheduling pages are not medical advice, diagnosis, treatment, or an emergency communication channel. If you have a medical emergency, call 911 or seek emergency care. For urgent clinical questions, contact the healthcare practice directly.
Your information and responsibilities
You agree to provide accurate information and to submit information only for yourself, your minor child, or another person when you are authorized to do so. You are responsible for keeping your contact information current and for reviewing appointment details before submitting.
Electronic communications
You consent to receive scheduling-related communications electronically, including through webpages, email, calls, and text messages, based on the information and permissions you provide. Electronic communications may include appointment confirmations, reminders, intake follow-up, schedule changes, and service notices.
FCC, TCPA, and text messaging consent
By providing your mobile number and checking the text messaging consent box, you authorize the healthcare practice, LeadSigma Scheduling, and their service providers to call or text you at the number provided using automated technology, prerecorded messages, or artificial voice where permitted by law. Messages may include appointment reminders, scheduling updates, intake follow-up, care-related logistics, and service notices.
Message frequency varies. Message and data rates may apply. Consent is not a condition of purchase or treatment; if you do not want to receive text messages, contact the healthcare practice directly to request scheduling support through another method. You may reply STOP to opt out of text messages and HELP for help where supported. Opting out of text messages does not prevent the practice from contacting you by other lawful means.
You represent that you are the subscriber or customary user of the phone number you provide and that you will promptly notify the practice if the number changes or is reassigned. Mobile opt-in data and text messaging consent will not be sold, rented, or shared with unaffiliated third parties for their marketing or promotional purposes.
HIPAA and practice notices
When information submitted through the scheduling service is protected health information, the healthcare practice's HIPAA Notice of Privacy Practices explains how the practice may use and disclose that information. LeadSigma Scheduling supports participating practices as a service provider or business associate, as applicable. Practice-specific notices control when they provide more specific requirements for that practice.
Acceptable use
You agree not to misuse the scheduling service, submit false or unauthorized information, interfere with security or operation, attempt to access data that is not yours, reverse engineer the service, or use the service for unlawful, abusive, or fraudulent activity.
Third-party services
The scheduling service may connect with healthcare practice systems, communications vendors, analytics tools, maps, file storage, or other third-party services. Those services may have their own terms and privacy practices.
Availability and changes
We may update, suspend, or discontinue any part of the scheduling service. We may also update these terms by posting a revised version with a new effective date. Continued use of the service after changes means you accept the updated terms.
Disclaimers and limitation of liability
The scheduling service is provided on an as-is and as-available basis to the fullest extent permitted by law. We do not warrant that the service will be uninterrupted, error-free, or that every appointment request will be accepted. To the fullest extent permitted by law, LeadSigma Scheduling is not liable for indirect, incidental, consequential, special, exemplary, or punitive damages arising from use of the service.
